This application can be used to apply for a Certificate of Relief from Disabilities or of Good Conduct from the New York State Board of Parole.

Individuals with one felony or fewer who served time in a New York State prison or were sentenced outside of New York State should apply for a CRD from the Division of Parole using this application form.

Individuals with more than one felony should apply for Certificates of Good Conduct for each felony from the Division of Parole, using this form, after the following waiting periods:
For Class A & B Felonies: Five years from completion of sentence
For Class C, D, & E Felonies: Three years from completion of sentence
